His first wife was American actress Jordana Brewster. He has two sons with her named Julian Brewster Form and Rowan Brewster Form. Their marriage ended with divorce in June 2021. He got engaged to another actress Alexandra Daddario on 2 December 2021. They got married in June 2022.

He began his career as an assistant producer to Tom Cruise starrer movie Top Gun producer Jerome Leon Bruckheimer alias Jerry Bruckheimer. He worked with famous producer Don Simpson who was partner to Jerry in productions. Form started by working in production of documentary film The Making of Crimson Tide in 1995. His next project in 1997 as a producer was movie Trading Favors.In 1998, he produced David Schwimmer starrer comedy movie Kissing a Fool. In 2001, he produced another comedy movie a Courteney Cox starrer The Shrink is in. then he moved to the genre that earned him fame. In 2003, he started producing horror movies in particular. His first was the remake of 1978 lasher movie with same name The Texas Chainsaw Massacre. In 2005, he produced remakes of supernatural horror movie The Amityville Horror. In 2006 he produced prequel to the 2003 remake of The Texas Chainsaw Massacre called The Texas Chainsaw Massacre: The Beginning. Later, he produced other horror movies including The Hitcher, The Unborn, Horsemen, Friday the 13th, Nightmare on Elm Street, The Purge, The Purge: Anarchy, The Purge: Election Year, The First Purge, and The Forever Purge, A Quiet Place, A Quiet Place Part II, Ouija, Ouija: Origin of Evil, science-fiction film Project Almanac, Teenage Mutant Ninja Turtles and Teenage Mutant Ninja Turtles: Out of the Shadows. In addition to that, he produced tv film Occult and three episodes of historical adventure series Black Sail. His long-term tv production is action drama The Last Ship. In 2001, he co-founded production company Platinum Dunes.
